Section I: Participant Consent
Please read the following, and submit the LA-GWEP consent form before completing this survey.
Study Title:
Louisiana Geriatrics Workforce Enhancement Program (LA-GWEP): Improving Quality of Integrated Primary Care among Persons Living with Dementia and Caregivers.
Study Purpose and Procedures:
The purpose of the LA-GWEP project is to improve health and wellbeing of LA residents living with dementia and their caregivers; and to improve dementia-friendly practice in LA primary care settings among health professionals in medicine, nursing, and social work.
LA-GWEP project procedures include the following 3 steps:
1. Participants will complete a pre-training survey that includes a pretest and a brief survey that collects sociodemographic information, including one potentially sensitive piece of information, namely, sexual orientation;
2. Participants next will receive a training that broadly focuses on health and well-being of persons with dementia and their caregivers
3. At the conclusion of the training, participants will complete a post-training survey that includes the same questions on the pretest and a training satisfaction survey.
Risks and Benefits
There are no foreseeable risks involved in participating in this study other than those encountered in day-to-day life. Although there are no direct benefits to participants, information gained from the study could improve health and wellbeing of, as well as enhance provision of integrated care to LA residents living with dementia and their caregivers.

Inclusion and Exclusion Criteria
To be included in the study, you must be a training participant attending a two-hour workshop presented by AS staff that provides an overview of dementia and dementia-friendly communication. There are no exclusion criteria. To participate in this study, you must meet the requirements of both the inclusion and exclusion criteria.
Privacy
The study is anonymous and you will not be asked to reveal your identity. Your privacy will be protected by LA-GWEP evaluators, who will collect your pretest-posttest data and training satisfaction feedback, and then group your information with the information collected from other LA-GWEP participants. The study results will be presented in reports, presentations, and publications; however, the names of LA-GWEP participants will not be included.

Right to Refuse
You may withdraw your consent to have your data collected and used at any time without loss of any benefit to which you might otherwise be entitled.
